A Message from Dr. Pranay Shah to Mehsana Patients

When a Mehsana couple walks into my consultation after three failed IUI cycles, the first thing I do is not calculate an IVF protocol. The first thing I do is ask what investigations were done before the IUI cycles started — and what was found.

Most of the time, the answer is: blood tests, basic ultrasound, and a semen analysis. Reasonable starting points. But they miss a category of structural problems that are present in a quarter to a third of women with unexplained infertility. Problems that a hysteroscope or a laparoscope would find in 30 minutes.

I am not saying this to criticise the clinics in Mehsana — I am saying it because the equipment and surgical expertise required for these evaluations simply isn’t available everywhere. That is what the 75 km drive is for. Not just to access a Class 10,000 IVF lab. To access a complete diagnostic evaluation that tells you, clearly and honestly, whether there is a structural reason your IUI cycles failed — and whether treating that reason before starting IVF gives you a meaningfully better chance.

I will not recommend an IVF cycle to a Mehsana patient when there is an uncorrected structural issue that would reduce its chance of success. That is not how I practice medicine.
