Most of the time, the answer is: blood tests, basic ultrasound, and a semen analysis. Reasonable starting points. But they miss a category of structural problems that are present in a quarter to a third of women with unexplained infertility. Problems that a hysteroscope or a laparoscope would find in 30 minutes.
I am not saying this to criticise the clinics in Mehsana — I am saying it because the equipment and surgical expertise required for these evaluations simply isn’t available everywhere. That is what the 75 km drive is for. Not just to access a Class 10,000 IVF lab. My HSG showed one blocked tube. Can I still try IVF, or does the tube need to be treated first?
It depends on what type of blockage, and whether the tube is simply blocked or whether it is a hydrosalpinx (fluid-filled damaged tube). A blocked tube alone does not prevent IVF — eggs are retrieved directly from the ovary, bypassing the tubes entirely. However, a hydrosalpinx is a different situation: the toxic fluid within a hydrosalpinx leaks into the uterine cavity and measurably reduces IVF success rates. Published studies show that a hydrosalpinx reduces IVF implantation rates by approximately 50% — and that salpingectomy (surgical removal of the hydrosalpinx) before IVF transfer restores success rates to normal. If your HSG or ultrasound suggests a possible hydrosalpinx, laparoscopic evaluation and surgical management before embryo transfer is strongly indicated. This is exactly the kind of finding that Dr. Shah would want to address before starting your IVF cycle.
How many trips to Ahmedabad will a full investigation + IVF cycle require from Mehsana?
